MO-Bolivar, Responsible for ongoing assessment, planning, implementation and evaluation of nursing care and education of patients with ESRD who have elected the Home Dialysis Program. Responsible for educating the patient on all aspects of home dialysis, including end stage renal disease, diet, medications, dialysis concepts, how to perform procedures, expected results, and any complications which may occur.
